When the teacher fills out the form, it will automatically be submitted to our Admissions Department.
2. Current Immunization Form 3231
Please email, fax, mail or deliver a copy of your child’s current immunization record (Form 3231) to our Lower School.

Please contact your child’s current school, requesting that your child’s school records be sent to Cornerstone. Ask for the current Immunization Form #3231 be included. Most school require a transcript release authorization. You can download that form here: Transcript Release Authorization Form
